The Spiritually blind are those that do not belong to God (John 8:44) They belong to satan.

They can't "see", "hear" nor "understand", much less believe the Gospel of Christ...1 Cor 2:7-14

Jhn 12:39-40 NIV - {39} For this reason they could not believe, because, as Isaiah says elsewhere: {40} "He has blinded their eyes and hardened their hearts, so they can neither see with their eyes, nor understand with their hearts, nor turn--and I would heal them."

They are as Paul describes believers before we were saved...Eph 2:1-5.
They are "dead" and need to be made alive, born again by the Grace of God.

Until God opens their heart they CAN NOT believe...Acts 16:14

Figuratively, blindness is used to represent lack of mental perception, lack of prevision, recklessness, and incapacity to perceive moral distinctions (Isa 42:16,18,19; Mt 23:16 ff; Jn 9:39 ff). Alex. Macalister

Dr. Warren Wiersbe sees Religious Stuper/Spiritually Ignorant as the symbol in 2 Cor. 4:4 and Isa 42:16, 18-19. I have heard many sermons about spiritual blindness, and the preacher would base such teaching on 2 Corinthians 4:3-4

3 	But if our gospel is hid, it is hidden to them that are lost: 	
4 	In whom the god of this world hath blinded the minds of them which believe not, lest the light of the glorious gospel of Christ, who is the image of God, should shine unto them.	
Wiersbe, like Michael, also include Revelation 3:17 in this category—

RE 3:17 Because thou sayest, I am rich, and increased with goods, and have need of nothing; and knowest not that thou art wretched, and miserable, and poor, and blind, and naked:...

One cause of spiritual blindness is greed (Numbers 22:20-23). Balak sent for Balaam again and again, tempting him with bribes but Balaam remained firm. Instead of saying with Daniel, “Thy gifts be to thyself and give thy rewards to another,” Balaam caught the bait held out and proved that he loved the wages of unrighteousness.

A clear lesson to be learned from this renowned man, Balaam, is to avoid being overcome by the besetting sin of greed (2 Peter 2:15). 

2 PE 2:15 Which have forsaken the right way, and are gone astray, following the way of Balaam the son of Bosor, who loved the wages of unrighteousness;
