The mark on the "forehead" are those who believe and their minds are controlled by the word and the wisdom of God. It is not a literal mark. Even in the book of Ezekiel the Lord asks to "mark" those who were groaning because of the evil that was going on in the temple. Those who remained faithful and odedient to the word of God in the mouth of the prophets were "marked-known or set apart." (Eze.9:4).

It is the same with those who reject the Word (the whole counsel of God), and are controlled by worldly systems and the thoughts of fallen nature of men. These are also "marked" that take on the mind and workings of the earthly-beastly systems that reject or are opposed to God. (Rev.13:16-17).
