How can I relate to my wife about a child born out of wedlock who is already an adult, and I just found out and want to establish a relationship with her?

With love and humility. Perhaps it would be easier for you to write it rather than say it. You can be there with her when she reads it. But just tell her the truth, building on whatever other self-disclosures you made along the way. How she responds will determine your next steps. She may be ok with your building your relationship without requiring her active involvement. I'm just guessing.
